We made a huge mistake leaving our mom at this place after a hospital stay. Physical therapy was good and had her walking with walker and minimal assistance the day before she was to discharge home. Her 9 day stay the nursing staff left her in soaked urine pull ups, clothes and bedding. I found her this way everytime I went there and that was everyday and I was the one who had to clean her and change her clothes. I complained but nothing changed. Constant medication errors and running out of her routine meds leaving me to drive home and bring her meds to her. Med aide gave certain medications too close together leaving her lethargic. On the morning of discharge I found her laying in a urine soaked bed and now has a another horrible UTI from this stay at Acell at Crystal Park. I WOULDN'T LEAVE MY DOG THERE!! They need to be investigated by the Department of Health and Human Services.

My dad was taken here after having a major hip surgery because he needed extended rehab. He was only there for about 8 to 10 hours and during that time he was treated so poorly and was ultimately transferred back to the hospital. He was brought to this facility around 4pm and when he asked for his pain meds, having broken his hip and major surgery only 3 days prior, he was told he could only have Tylenol until he could be "assessed" sometime the following day. It took over an hour for anyone to respond to his call light. When he would ask questions or ask for help he was told "that's not my job" by the CNA. I could go on but it gets a bit personal so I won't. This place has a poor rating for a good reason. Steer clear!
