On July 30, 2021, I placed my husband in Carrollton Autumn Leaves memory care facility. He was rushed to the ER twice the first week he was there. One reason Autumn Leaves was chosen is they said they had hall and common area monitoring. He was a Vietnam veteran and they said there were two other vets there as well. Tuesday morning around 1:30 am, he had an "unwitnessed" fall they said was not picked up on the monitor. The following Friday around 4 pm, I got a call to let me know he had fallen again but from a dining room chair and was being taken to the same ER. After being released that time, I took him home and the next day I went over to Autumn Leaves, packed his belongings and returned the key. I sent Lucy, their executive director, an email documenting the reasons why and told her I no longer felt they could safely take care of him and asked for the return of the remaining room and board I had paid. She never answered any of the subsequent emails or calls from me so I filed a civil suit for the unused portion of R&B + Community Funds and won the judgment (Sept 1, 2023). As of this day (July 23, 2025), they have refused to acknowledge or even answer any of the post judgment appeals I have sent. My experience Autumn Leaves and the owners, J&M Family Management, has been terrible and very disappointing. Mrs. J. My mother has been at Autumn Leaves for 13 years and loves it. Although it has changed hands several times during those years, it has continued to improve. The current owners are responsive and many of the staff have been there for years and remember when Mom first moved there. She started there with a period on the skilled nursing floor, went back home for less than a year and then moved permanently into assisted living. Management and staff have developed a culture of working together to provide a safe and nurturing environment. Glenn in facilities has always responded quickly whenever the inevitable issues arise. Kristine, Carrie and Edmund call me when needed to keep me apprised of Mom's situation. Perfect? No, but deserving of an A, definitely.

We are satisfied with her room and the facilities, and very satisfied with the caregivers who tend to her regularly. I would not hesitate to recommend their assisted living facility.
