Stayed here for a total of 2 months and it's a lock down facility set smoke times not allowed to leave freely if in stepping stones

I wish I could give it zero stars. My mother wasn’t safe there. They didn’t follow fall precautions and she fell twice, hitting her head and getting a hematoma. They moved her into a dirty room they didn’t bother cleaning or disinfecting before her arrival. The place is filthy and everything is damaged. Her oxygen tank popped and caught fire in her room. Then they gave her another broken oxygen tank. I reported this terrible facility to the state!! Don’t bring your loved one here. You’ll regret it.
