Care Pavilion Nursing and Rehabilitation Center

Care Pavilion is a facility that no one should have to live in. The staff appear untrained in safety issues, unprofessional, and often condescending. During my visit, I witnessed serious problems that make this place unsafe and unsuitable for vulnerable residents. There was only one staff member supervising 36 adults with mental health challenges. I was able to visit my family member without showing any ID or proof of who I was—a major security risk. If I had wanted to bring drugs or other harmful items inside, I could have easily done so. My family member, who lives with substance use and mental health challenges, is not safe in this environment. I saw a resident walking around in a hospital gown with no underwear, which was degrading and unacceptable. Other residents were throwing glass dishes at each other, and one was swinging a cane at another person. The facility is unclean, poorly maintained, and lacks access to fresh air. This facility fails to provide even the most basic standards of care and dignity. Please do not place your loved ones here—it is unsafe and neglectful.

Some of the staff is excellent and do not deserve the bad review but most are horrible. It takes 30-45 minutes, sometimes over an hour for someone to answer the call bell. Medication is late, the food is half horrible and half edible if you provide your own seasoning. They are only collecting your money while doing the bare minimum.

Well, something has changed for the better! the elevator was clean! It did not smell! The floor wasn't sticky under my feet today! The dining hall had a brighter look to it and different tables. Now if they can get more chairs and a designated place for family to come and sit with their love ones would mk my day!!
