Anger does not describe the emotions that I am feeling. We brought my father to this place for rehabilitation he had his leg amputated. He was there for a total of five days and in that time the staff kept his food and water out of his reach and the call button was not accessible to him, they said he was to needy. ( he is 86 and cant move ?) The fifth day started with him falling on the floor and nobody could tell me how long he was there I was never even notified in the afternoon he started with diarrhea and when we got there he was soiled and going every 10 minutes we called 911 to get him the help he needed he is still in the hospital and severely dehydrated. Do not bring your loved ones here they are very incompetent and you will regret it. There is no compassion here from the staff, this place should be shut down. They get a minus five star rating from me.

Unfortunately Forest is not Compassionate at all! My grandfather had to have his leg amputated and was sent to Forest Street for rehabilitation. The PT was to move him from his bed to wheelchair and that was it. He fell from his bed and sitting on the floor for over 2 hours before anyone had checked on him. When I arrived at Forest Street he was very dehydrated and his water and dinner was out of reach along with his call button. They told me that he had fallen out of his bed and had no contact information to call anyone. While I was waiting for the ambulance I found his contact sheet with 4 family member's phone number and all of our information and gave it to the lady at the front desk and she had no response for not contacting us. Jess and Albert were very Compassionate CNA's but cannot be at Forest Street every hour of the day. The entire facility is very dirty. I would never recommend for you to take someone to this facility for care or rehabilitation. Honestly Forest Street needs to shut down.
