About Fountain View Subacute and Nursing Center
Fountain View Subacute and Nursing Center brings together skilled nursing, rehabilitation, and hospice care in the heart of Los Angeles’s Little Armenia neighborhood. While the facility itself may feel a bit dated, what stands out is the dedication of many staff members—especially CNAs and respiratory techs—who are praised for their attentive, respectful care and willingness to answer questions. Residents and families appreciate the 24-hour support, emergency alert system, and a range of amenities like movie nights, walking paths, and an arts room. The community’s location offers easy access to local pharmacies, cafes, and parks, making it a practical choice for families seeking comprehensive care in a culturally rich area.
Our Care Approach
Fountain View’s care philosophy appears centered on providing round-the-clock medical and personal support, with particular strength in respiratory and daily living assistance. While nursing and medication management are standard, the hands-on care from CNAs and techs is frequently highlighted as a strength. However, administrative and social work support receives less consistent praise, indicating room for improvement in care coordination.
Family Experience
Families who have positive experiences highlight staff responsiveness, good communication, and feeling that their loved ones’ needs are a priority. Others express frustration with administrative hurdles, difficulty getting information, and concerns about the overall environment. Several note that regular visits and direct interaction with care staff improve their satisfaction.

I had a bad experience there. I was supposed to stay for 2 weeks. I ended up staying ther 3 weeks. The administrator didn't want me to see the doctor to be released. So that I could check out the facility. I had enough. The administrator didn't want me to leave. Telling me we can take you to your other doctor appointments. This was not acceptable by me. I felt they were trying to keep me there for ever. So I had to leave against doctors orders. Don't send your loved ones there.

Clean facility with great staff and leadership who truly care about the patients, and patient outcomes.

I feel so blessed that my mother receives the care, attention and respect she deserves. Everyone is pleasant, professional and responsive. Whatever concern or need I have, someone always communicates with me and addresses those needs or concerns. I feel that my mother's care is of the utmost importance to all at Fountain View. I am truly indebted to all of you. I would like to note that the CNAs who assist my mother on a daily basis are my heros (especially Lola).
