I highly recommend AGAINST choosing fountains as a place of care for your loved one. This agency over-sedated my grandfather who was only there for rehab regarding a catheter. They saw his age and basically tried to kill him off. He was very competent, and coherent before getting there. They disconnected his room phone on multiple occasions. Did not set his bed up properly, this was problematic due to the fact he needed to spit phlegm up when he coughs, he could have choked to death. They did not ensure he was hydrated while giving him multiple medications, did not check his blood pressure. Gave him a bed pan when they could have taken him to the bathroom. Lied about him receiving physical therapy on the weekend when PT does not work on the weekend. They lied about bathing him and so much more. None of the residents were up and alert, they were all over-sedated and the place was quiet as ever. Everyday there was new staff and no communication between nurses and aides. The place should be investigated immediately. Do not choose Fountains Transitional Care Center for care if you are in Cincinnati. I repeat, DO NOT. The place has gone completely down hill since it’s opening. Short staffing, Negligence and Over-sedation just to name a few additional issues. We also believe that he fell and it was undocumented. The staff tried to deny this, and blamed it on his dementia. He dementia is not full blown by any means and it is only connected to recalling events related to his memories when he drove trucks. Now he is back in the hospital due to dehydration because they did not ensure he was properly cared for. Again, do not send your loved ones here.

The staff here is so friendly and helpful. Smiling and caring all the time. They actually add to your happiness in a very unpleasant situation. Mother-in-law was there for months and all we ever heard was how wonderful "Miss Patti" was. That attitude prevails and shows how kind hearted the people here are.
