About Landmark Of Louisville Rehabilitation And Nursing
Landmark Of Louisville Rehabilitation And Nursing is one of Louisville’s largest skilled nursing facilities, recognized for its long-standing presence and significant capacity for Medicare and Medicaid residents[1][2]. The community operates around the clock, providing nursing care, medication management, and daily living support, especially for those who need extra help getting around. While the facility lists a wide range of amenities—from fitness and arts rooms to scheduled activities and outdoor spaces—reviews and official ratings reveal serious concerns about care quality, cleanliness, and resident dignity[1][3]. Families should note that, despite the extensive amenity list, actual resident experiences and state inspections highlight significant challenges, including staffing shortages and a high number of health violations[1][3]. Landmark’s location in Germantown offers convenient access to local hospitals, pharmacies, and cafes, but the community’s reputation for care and communication with families remains a critical consideration.
Our Care Approach
Landmark Of Louisville provides round-the-clock nursing care and assistance with daily activities, but reviews and inspection reports indicate that staffing shortages and inconsistent care standards are ongoing issues[1][3]. The facility’s approach to resident rights and dignity has been questioned, with some residents not receiving preferred foods or respectful treatment during meals[3]. Families should be prepared to monitor care closely and advocate for their loved ones.
Family Experience
Families often describe frustration with staff communication, especially following critical incidents or a loved one’s passing. Several reviews mention a lack of empathy or follow-up from leadership. Visits reveal concerns about resident well-being, with some families feeling compelled to advocate aggressively for basic care. Positive family experiences are rare and typically focus on individual staff members rather than the facility as a whole.

Medical Care Services
24-hour nursing care, medication management, and health monitoring are provided, though staffing and care consistency are noted concerns[1][3].
Personal Care Services
Assistance with bathing, dressing, and daily living activities is available, but reviews suggest lapses in basic care and dignity for some residents[3].
Specialized Care
No memory care or behavioral health services are listed. Therapy services are mentioned positively in a few reviews, but overall specialized care is not a highlighted strength.

Living Environment
Cleanliness & Maintenance
State inspections and family reviews cite significant problems with cleanliness, including soiled rooms, broken outlets, and poor upkeep in common areas[3]. Multiple shower rooms and ice machines were found in unsatisfactory condition during inspections.
Atmosphere & Community Feel
The social environment appears strained, with some residents feeling scared or neglected, while a few found camaraderie with staff. Scheduled activities and amenities are listed, but review themes suggest these may not consistently translate to a positive daily experience.
Safety & Security
While the facility is staffed 24/7 and has an emergency alert system, reviews and inspection reports raise concerns about resident safety, including falls and delayed response to needs[3][4].

My mom passed away there. It's been almost 2 months, and we haven't gotten a letter, text, call, email with condolences, sympathy to the family...nothing. To me, this is very disrespectful to me, rude and unprofessional. If I go to a nursing home later in my life, it WILL NOT be there.

Femm M. provided the first review, and I investigated their post. They are 100% correct. Lottie Kleire posted the very first review of Eastway on Facebook. However, Ms. Kleire is currently employed at Eastway under the name of Lottie Nicholas as Director of Nursing. On Thursday, August 7, 2025. I had a meeting with Ms. Nicholas to address the undeniable lack of care for my grandmother. Ms. Nicholas (Kleire) contacted the Kentucky Cabinet of Family and Children Services to receive verification that she could speak with me concerning my grandmother. After they provided permission for Eastway to speak with me, she did not provide any information and even suggested that I provide materials needed to care for the grooming of my grandmother (a foot bath). I immediately contacted Adult Protective Services, Jefferson County District Judges, and KCFCS. If Ms. Nicholas takes the time to deceitfully create a public review about her facility, what else is she capable of denying the truth about? This senior care facility requires new leadership. I had a meeting with Mr. Kellogg in 2023, and he was not helpful. His daughter Katelyn Kellogg posted a Google review and praised her father, without stating that she is related to the director. I am on a personal journey to ensure Eastway takes accountability. I am not just simply making a review. This is the beginning of Eastway being held accountable. I have visual documentation.

Did you have a horrible childhood? Do you seek revenge on your parents? If so, then this is the place for you. If I could give -5 stars I would and that’s still not enough negative. There is nothing good about the place at all. I can’t think of one good thing to say. Just grateful I got my mom out of there before they killed her. Please do not send your loved ones there.
