I have my mother in this facility for 2yrs and 5 months.the beginning was great my mom is improved so much with the care of the nurse in the sub acute station. But it was an on and off complaining to see over here I had to leave my job for 3 months just to make sure she was observed I was at the facility every other day.after I went back to work it got worse she had blisters, bruises,she even took off her breathing tube multiple times due to itching nasty and a bad smell came out of her throat.she is now in a different area of the facility were the nurse probably only see them once a day. She is always dirty, breath smell horrible, she has multiple blankets and personal items were stollen even her fan that was bought with our money is missing and we have asked multiple times for it and it has been told to us by nurses that a manager has it in there office. I have been asking to transfer her out to a closes facility to us and they keep ignoring us this facility has seen over 3 management changes since my mom been here.And it is still the same. Please make sure you check were your loved ones are placed before you sign any paperwork. I made the mistake of not making sure this place was safe.

Any tips on reaching anyone? It's been 2 days. I've been hung up on 4 times. Placed on hold 4x for 20 minutes. The people who do answer, including Yolanda, have been friendly and helpful but that's once in 15 attempts.
