Our family was searching for an assisted living facility for my mom, who is in the middle stages of Alzheimer's Disease. After doing a ton of research, Urbana Place became one of our top choices. Last year, I reached out to Shelly, Director of Marketing, to schedule a tour. Shelly was very caring and friendly, and very informative. She took the time to listen to our story and shared valuable insights from her own experience, which provided comfort during this very difficult decision-making process. The facility itself went above our expectations. The apartment options were great, and the range of services they offered was exactly what we were looking for. After visiting other facilities and considering the timing and options for my mom's move, we ultimately returned to Urbana Place. When we decided it was time, they had the ideal apartment available, and Shelly made the transition seamless and stress-free. Throughout the entire process, Shelly was outstanding - responding promptly to every email and call and addressing all my questions with patience and care. She really went above and beyond. Since moving in, my mom has had a positive experience. The staff and residents have been very welcoming and supportive, helping her adjust and feel comfortable with this significant change. The variety of daily activities has been wonderful for my mom, who enjoys staying active and engaged with others. Overall, our experience with Urbana Place has been only positive. We are confident and hopeful that this will continue to be the best fit for my mom as she settles in and moves forward.

WARNING for Families: Do NOT move your loved one(s) into Urbana’s Memory Care. If moving in, do NOT sign their optional arbitration agreement – Shelly (Urbana Marketing Director) says it benefits all parties, but it only benefits Urbana, NOT families. Multiple lawsuits are happening against Urbana. Mom lived in Urbana’s Memory Care (Legacy Court) Feb 2024-Jun 2025. On the surface, Urbana appears to be a great place for residents. Unfortunately, this is not the case at all. My sister & I moved our mom out because of the ongoing negligence of her daily care in Memory Care. Our mom was repeatedly mentioned as one of the sweetest & easiest-to-care-for residents, yet the aides would consistently NOT complete her cares or NOT complete them correctly. We installed 3 cameras & hired external caregivers to enforce Mom’s cares getting done. Biggest issues (among many): * INFECTION CONTROL: the aides don’t wash their hands before/between cares, they either don’t wear gloves or wear unsanitary gloves pulled from pockets, and they’ll use the same gloves for toileting then oral care. When we pressed leadership about why this issue isn’t being resolved after several months, they quoted their protocol for glove usage being for the safety of their Urbana aides (not for the residents), which explains why they’re allowing this issue to continue. * SAFETY: the residents have been left unsupervised during mealtimes & in the common areas, which poses severe safety risks due to choking, falling, incurring injuries left untreated, fighting between residents, etc. Required safety checks aren’t completed every 1-2 hours, and residents are left alone for several hours without anyone putting eyes on them. * TOILETING: the aides repeatedly skip required toileting & brief changes, leaving residents sitting & soaked in their own urine for several hours at a time. *ORAL CARE isn’t done correctly; the list goes on… After tracking 1mo of our mom’s care, Urbana aides completed only 30% of her required services yet reported them as 100% complete, and Urbana still charged 100% too, which is financial fraud. After pushing hard Oct 2024-Jun 2025 for Lifespark/Urbana leadership to fix the issues in Urbana’s Memory Care, with minimal/no lasting success, it’s clear things will never truly improve at Urbana. I interacted often with Shaelee (Urbana Executive Director), Elizabeth (Urbana Director of Nursing), & Jennifer (Lifespark Regional Director of Nursing), and I consistently experienced gaslighting, dodging questions, providing flowery & ambiguous answers w/ no value or clarity, and just outright lying about situations that various individuals have witnessed as occurring. I interacted with Emily (Lifespark Regional Director of Operations, Shaelee's supervisor) who is very retaliatory. I interacted with Katie (Lifespark VP of Operations, Emily's supervisor) who expresses contempt & is also a negative contributor of the existing issues, as she tries to illegally charge extra fees & make up rules which are opposite of what is stated in Urbana’s documentation. I interacted with Amanda (Lifespark Corporate Compliance Officer, Katie's supervisor) who recently (& suddenly) resigned without any communication to families she was actively dealing with; she was helpful in some ways, but she was unfortunately still part of Lifespark’s systemic issues. A family was offered financial reimbursement from Lifespark for care not delivered to their mom when she was in Urbana’s Memory Care, yet Lifespark also stated that Urbana is taking zero responsibility & requested that a nondisclosure agreement (NDA) be signed to keep Lifespark/Urbana's appearance clean in the public eye. I solidly confirmed that all the issues not only run across ineffective leadership at Urbana but also up through ineffective (and, sometimes retaliatory & even corrupt) leadership at Lifespark. When the systemic issues run all the way up through the top of leadership, there is no hope for things to actually improve. My best advice is to stay away from Urbana & all Lifespark facilities.
