When paid senior living is not an option, Illinois offers health coverage, in-home care, meals, and caring people who will walk you through your choices.

Start here for most families
Illinois works hard to help seniors stay out of nursing homes. Through the Department on Aging and Medicaid, these include:
Ask about the Community Care Program and other in-home care options.
Ask about Medicaid long-term care, including the Persons who are Elderly Waiver and Supportive Living Program.
Illinois Medicaid, run by the Department of Healthcare and Family Services (HFS), gives free or low-cost health coverage. For seniors it includes doctor visits, hospital care, prescriptions, and long-term care — both in nursing facilities and through services that help you stay home.
A note on eligibility: Illinois looks at your income and, for long-term care, your assets. The limits change every year, and there are protections for a spouse who lives at home. Do not assume you make "too much" before you check — many people are surprised to qualify. Call to confirm your real numbers.

PACE bundles all your medical care, prescriptions, therapy, and social support into one coordinated program so you can keep living at home.
Illinois began rolling out PACE in 2024, with programs designated in several regions including West Chicago, South Chicago, southern Cook County, Peoria, and East St. Louis. Availability is still growing.

The Senior Health Insurance Program (SHIP), run by the Illinois Department on Aging, offers free, unbiased Medicare counseling. Counselors do not sell insurance and earn no commissions.
They can help you understand Medicare and what it covers, compare drug and Medicare Advantage plans, apply for programs that lower your costs, and handle billing problems and appeals.
